Our process to design detailed service delivery models In designing the detailed service delivery model we are:
- Gathering information from within the health service, from across Australia and from around the world to makes sure our practice is the based on the best
evidence we can find
- Naming what we want to include
- Documenting our models
- Implementing as much of each model as we can
before the new hospital opens
- Monitoring and evaluating what we do to make sure it all works or changing what does not work.
All service delivery models show that:
- Patients and carers actively participate in decision making regarding their care
- Care is co-ordinated, designed and delivered collaboratively across the continuum of care
- All models have a holistic health approach (looking after the whole person not just their ‘hip’)
- The staff profile gives us the best patient outcomes using a team approach
- Innovative solutions are supported with services based on best available evidence
- We can ‘do it once' with technology
- Our work is standardised with repeatable outcomes
- Care is delivered in the best timeframes
- We can measure what we do in a meaningful way
- Patients won’t be moved around when they don’t need to be
- Our role is defined for our local and regional communities.
